Based on interview and observation, the licensee did not comply with the section cited above in one out of three staff members who were missing the required immunizations. During record review, LPA observed that staff 2 was missing immunization against pertussis and measles. This poses a potential health, safety or personal rights risk to persons in care.

During file review, LPA observed that staff 1’s mandated reporter certificate expired on 03/26/2025. Per staff 1, they cannot remember when they completed the training. This poses a potential health, safety or personal rights risk to persons in care.
