Based on observation and interview, the licensee did not comply with the section cited above. At approximately 10:08am, LPA observed that there was no fence or barricade blocking access to the stairs. Per licensee, they block the stairs with a net, but only when children arrive to the facility. Once they arrive the net is remove. This is a potential safety and personal rights risk to persons in care.

Resolution: Per licensee, they will write a declaration stating that the barricade will remain at the foot of the stairs while children are present and email declaration to LPA J. Culbert by plan of correction date.

Based on record review and interview, the licensee did not comply with the section cited above. During record review, LPA did not observe record of fifteen-minute sleep checks. Per licensee, they have a form to log sleep checks, but they do not use it. This is a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, they will start to document infant sleep checks every fifteen-minutes and email LPA J Culbert a copy of the log on the plan of correction date.

Based on record review and interview, the licensee did not comply with the section cited above in 1 out of 2 licensees did not have a mandated reporter certificate. Per licensee (Wilhemina) they were not aware that Josefa needed to complete the mandated reporter training. This poses a potential health, safety, or personal rights risk to persons in care.

Resolution: Per licensee (Wilhemina), Josefa will complete the training and email a valid mandated reporter certificate to LPA J. Culbert by plan of correction date.

Based on observation , the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a safety gate by 4/15/2022 then barricade the stairs access to the second floor then send LPA photo as proof in order to clear this citation.

Resolution: Licensees agrees to renew the mandated reporter training then submit the department the copy by 5/13/2022 in order to clear this citation.
