Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #1 had an LIC 627 Consent for Emergency Medical Treatment on file however not signed by the parent/legal guardian. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have the parent/legal guardian of the child sign the consent (LIC 627) form and the licensee also agrees to submit a copy to the Riverside Child Care Regional Office by 12/12/2025.
Based on record review, the licensee did not comply with the section cited above. The licensee's assistant did not have a complete record of the required immunization. File only had record of MMR. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have her assistant obtain the additional immunization. Licensee also agrees to submit record of her assistant's Tdap/Pertussis to the Riverside Child Care Regional Office by 10/31/2022.
Based on observation and licensee admission, the licensee did not comply with the section cited above. The licensee serves 2% milk to the children two years of age or older. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to purchase 1% milk for the children and provide proof of purchase to the Riverside Child Care Regional Office by 10/31/2022.
Based on record review, the licensee did not comply with the section cited above. The licensee only completed the General portion and not the section required by AB1207. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the training and submit proof of completion to the Riverside Child Care Regional Office by 11/30/2021.
