Based on record review, the licensee did not comply with the section cited above. In reviewing children files, the LIC 700 Identification and Emergency form, was missing in the files for child #4, #5, and #6. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have parent/legal guardians complete the form (LIC 700) to place in the children files. A copy is to be submitted to the Riverside Child Care Regional Office by 6/27/2025.
Based on record review, the licensee did not comply with the section cited above. In reviewing staff/assistant record/file, LPA did not observed a Health Screening/TB Test on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have staff/assistant get a Health Screening/TB test and submit proof to the Riverside Child Care Regional Office by 7/11/2023.

Resolution: Licensee agrees to obtain all required documents, to complete the child's file, and submit a copy to the Riverside Child Care Regional Office by 8/11/2022.
