Based on record review, the licensee did not comply with the section cited above. In reviewing and requesting records for the licensee, proof of required training was not available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and submit proof of completion to the Riverside Child Care Regional Office by 6/20/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records/files, child#1 did not have the required immunization records on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required records and submit a copy to the Riverside Child Care Regional Office by 6/20/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records/files, child#1 did not have the required form (LIC 995A) signed by the parent/legal guardian, on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide the required form to the parent/legal guardian to complete and sign. A copy is to then be submitted to the Riverside Child Care Regional Office by 6/20/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #9, did not have proof of immunization on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a copy of the required record/form and submit a copy to the Riverside Child Care Regional Office by 7/29/2022.

Child #2, 3, 4, 5, 9, & 10, did not have a signed LIC 995A available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Child #2, 3, 4, 5, 9, & 10, did not have an LIC 700 or LIC 627 available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain copies of signed LIC 700 and LIC 627 forms and submit copies to the Riverside Child Care Regional Office by 7/29/2022.

Based on interview and record review, the licensee did not comply with the section cited above. There are several children in care that attend Elementary School without proof of enrollment as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of child's enrollment in elementary school and submit copies to the Riverside Child Care Regional Office by 7/29/2022.

Based on record review, the licensee did not comply with the section cited above. The licensee submitted an altered Preventative Health and safety certificate in lieu of completing the updated course. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to enroll in a Preventative Health and Safety course and submit proof of enrollment to the Riverside Child Care Regional Office by 7/22/2022.

Based on record review, the licensee did not comply with the section cited above. The licensee had a total of 12 children in care, however 6 of them did not have a file for review. Child #1, 2, 3, 4, 5, 9, & 10, did not have an LIC 282 available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain copies of signed LIC 282 forms and submit copies to the Riverside Child Care Regional Office by 7/29/2022.

Based on interview and record review, the licensee did not comply with the section cited above. The licensee did not have proof of documenting every 15 minute check for Child #1. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to create an Infant Sleep log and submit a copy with entries, to the Riverside Child Care Regional Office by 7/29/2022.

Based on record review, the licensee did not comply with the section cited above. The licensee relocated from one facility to another and in doing so, submitted an altered document to meet the requirements for relocation and licensure. The licensee altered the date of completion from 4/28/07 to read 4/28/20. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to submit a statement explaining why an altered document was submitted and regarding the seriousness of altering documents. Also submit a receipt of enrollment into a Preventative Health/Safety course. Statement and receipt to be submitted by July 22, 2022 to the Riverside Child Care Regional Office.

Based on record review, the licensee did not comply with the section cited above. The roster that was reviewed had several names of children missing and/or information not updated as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to updated the facility roster and submit a copy to the Riverside Child Care Regional Office by 7/29/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing the fire drill log, the last drill was conducted on 7/22/2021. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill, log it, and submit a copy of the log to the Riverside Child Care Regional Office by 7/29/2022.

Based on record review, the licensee did not comply with the section cited above. The licensee and her assistant have not completed and/or renewed the Mandated Reporter Training in over two years. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and submit a copy of the certificate to the Riverside Child Care Regional Office by 7/29/2022.
