Based on record review, the licensee did not comply with the section cited above. The facility roster was not up to date due to missing names/information of some of the children currently present. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to update the facility roster and submit a copy to the Riverside Child Care Regional Office by 7/31/2023.
Based on record review, the licensee did not comply with the section cited above. In reviewing children records, LPA did not observe the required form (LIC 9150) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have the parents sign/complete the required form (LIC 9150) and place the correct portion in each child's file. Licensee also agrees to submit copies of the completed forms to the Riverside Child Care Regional Office by 7/31/2023.
