Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #4, did not have a signed LIC 627 form (Consent for Emergency Medical Treatment) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a signed form (LIC 627) and submit a copy to the Riverside Child Care Regional Office by 4/4/2024.

Based on record review, the licensee did not comply with the section cited above. Child #3 did not have a record on file regarding the 15 minute checks that are required. This poses/posed a potential health, safety or personal rights risk to persons in care. Please note that this is a repeat violation/citation from previous inspection on 4/7/2022. A civil penalty is being assessed in the amount of $250.00

Resolution: Licensee agrees to start logging the 15 minute checks conducted, while the infant is sleeping. A copy of the log is to be submitted to the Riverside Child Care Regional office by 2/24/2023.

Based on record review, the licensee did not comply with the section cited above. The licensee did not have proof of conducting fire/disaster drill on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill, log it, and submit a copy of the log to the Riverside Child Care Regional office by 4/21/2022.
