Based on record review, the licensee did not comply with the section cited above. In reviewing and requesting records for licensee and assistant, proof of required training was not available for review, for the licensee and assistant. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and have the assistant complete it as well. Proof of completion to be sent to the Riverside Child Care Regional Office by 6/20/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing assistant's file, LPA was not able to review the required health screening/tuberculosis records, as they were not available. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required records and provide copy to the Riverside Child Care Regional Office by 6/27/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records/files, LPA was not able to locate the required 15 minute check/log in the file of child#6, for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to begin documenting the checks conducted every 15 minutes, when the child/infant is sleeping. Licensee agrees to submit a copy of log to the Riverside Child Care Regional Office by 6/20/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, LPA was not able to locate the required LIC 9227 Sleeping Plan form in the file of child#1, for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have the parent/legal guardian complete the required form and the licensee will forward a copy to the Riverside Child Care Regional Office by 6/20/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing assistant's file, LPA was not able to review the required immunization records, as they were not available. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required immunization records and provide copy to the Riverside Child Care Regional Office by 6/20/2025.

Based on observation and record review, the licensee did not comply with the section cited above. Licensee was providing care and supervision for eight children (one infant, one school age, six preschoolers), without an assistant. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to contact assistant during the inspection to be present to meet ratio. Licensee also agrees to submit a written plan as to how the facility will meet compliance, at all times, with the cited section. Plan to be submitted to the Riverside Child Care Regional Office by 5/17/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, child #7 did not have the required records on file for review by the Department. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required records and submit copies to the Riverside Child Care Regional Office by 5/30/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, child #7 did not have the required records/forms (LIC 700/ID and LIC 627/Consent) on file for review by the Department. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have parent/legal guardian complete the required forms to have in child's file for review. Copies of the completed forms to be submitted to the Riverside Child Care Regional Office by 5/30/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, child #7 did not have the required form (LIC 9227 Sleep Plan) on file for review by the Department. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above. Licensee was not logging the 15 minute checks conducted while the infant was sleeping/napping. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document 15 minute checks as the infant sleeps. A copy of the completed log is to be submitted to the Riverside Child Care Regional Office by 5/30/2024.

Based on record review, the licensee did not comply with the section cited above. According to the facility log, the last drill was conducted on 12/5/2022. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a drill, log it, and submit a copy of the log to the Riverside Child Care Regional Office by 6/22/2023.

Based on observation, the licensee did not comply with the section cited above. During the walk through of the backyard, LPA observed a small citrus tree that has thorns on the branches. Due to extreme weather the children are currently not playing outdoors. Therefore, this poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to place a barrier around or remove the tree. Licensee agrees to submit proof of correction to the Riverside Child Care Regional Office by 6/22/2023.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, there were two infants (Child #5 & #10) in care that did not have a Sleep Plan (LIC 9227) available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete Sleep Plans, with the assistance of the parents, and submit a copies to the Riverside Child Care Regional Office by 8/18/2022.

Based on observation, the licensee did not comply with the section cited above. The licensee began to place children to nap/sleep in a room that was previously listed as "off limits". This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit an updated facility sketch (LIC 999) showing the rooms that are now being utilized for child care and those that are off limits. Copy to be submitted to the Riverside Child Care Regional Office by 8/18/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, there were three infants (Child #5, #10, & #12) in care that did not have a Sleep Log/Chart available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to compose and begin documenting the 15 minute checks, as required. Copies of the logs/charts are to be submitted to the Riverside Child Care Regional Office by 8/18/2022.

Based on record review, the licensee did not comply with the section cited above. The last time the licensee completed the required training was over two years ago (6/24/2020). This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and submit a copy of the certificate of completion to the Riverside Child Care Regional Office by 8/18/2022.

Based on record review, the licensee did not comply with the section cited above. The licensee provided an expired CPR/1st card for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to enroll in an EMSA approved CPR/1st aid course and submit proof of enrollment to the Riverside Child Care Regional Office by 8/18/2022.
