Based on record review, the licensee did not comply with the section cited above. In reviewing and requesting records for licensee and assistant, proof of required training was not available for review, for the licensee and assistant. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the required training and have the assistant complete it as well. Proof of completion to be sent to the Riverside Child Care Regional Office by 6/20/2025.
Based on record review, the licensee did not comply with the section cited above. In reviewing assistant's file, LPA was not able to review the required health screening/tuberculosis records, as they were not available. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain the required records and provide copy to the Riverside Child Care Regional Office by 6/27/2025.

Licensee also agrees to submit a written plan as to how the facility will meet compliance, at all times, with the cited section. Based on observation, the licensee did not comply with the section cited above. During the walk through of the backyard, LPA observed a small citrus tree that has thorns on the branches. Due to extreme weather the children are currently not playing outdoors. Therefore, this poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to place a barrier around or remove the tree. Licensee agrees to submit proof of correction to the Riverside Child Care Regional Office by 6/22/2023.

In reviewing children records, there were two infants (Child #5 & #10) in care that did not have a Sleep Plan (LIC 9227) available for review.
