HERRARTE FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above. Upon arrival, LPAs observed 9 children in care with in the licensee's assistant present. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During the inspection, the licensee and another assistant arrived, and at that point were in compliance. The licensee agrees to submit a written plan regarding how they will assure compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 8/1/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records/files, child#1 and child#3, did not have the required LIC 995A form, signed and in the file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide the parent/legal guardian with the required form (LIC 995A) for completion. Once the form has been completed/signed, the licensee agrees to submit a copy to the Riverside Child Care Regional by 8/15/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #1, did not have the required LIC 627 consent form signed in their file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant agreed to obtain the required form (LIC 627) and submit a copy to the Riverside Child Care Regional Office by 8/30/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #1, did not have the required immunization record in their file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant agreed to obtain the required records and submit a copy to the Riverside Child Care Regional Office by 8/30/2024.

Based on record review, the licensee did not comply with the section cited above. There was an infant (child #2) in care that did not have the required documentation (LIC 9227) on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to have the parent/legal guardian complete the required LIC 9227 form. Licensee also agrees to submit a copy of the LIC 9227 to the Riverside Child Care Regional Office by 9/15/2023.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records LPA did not observed the required Safe Sleep Logs for Child #1 and #2. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document the 15 minute checks as required by the cited regulation. Licensee also agrees to submit copies of the logs to the Riverside Child Care Regional Office by 9/15/2023.

Based on observation and record review, the licensee did not comply with the section cited above. LPA determined that the licensee was caring for five children under the age of 2 years olds (infants). This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee has agreed to terminate care, temporarily, for one of the children, in order to be in immediate compliance. Licensee also agrees to submit a written plan on how the facility will assure compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 9/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, LPA did not observed required immunization records on file for Child #1. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required records and submit a copy to the Riverside Child Care Regional Office by 9/30/2022.
