Based on record review, the licensee did not comply with the section cited above. Licensee and assistant did not renew the required training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to renew training and have assistant do so as well, and submit proof of completion to the Riverside Child Care Regional Office by 3/7/2025.
Based on record review, the licensee did not comply with the section cited above. Licensee has not renewed the required Mandated Reporter Training. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to complete the required training and submit a copy of the certificate to the Riverside Child Care Regional Office by 2/28/2023.
Based on observation, the licensee did not comply with the section cited above. In touring the home, LPA observed toothpaste in the bathroom assigned to the children, also in the backyard cans of Raid (insecticide), gardening tools with sharp edges, floor cleaners, gasoline can, amongst other items. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to make the backyard off limits, temporarily, until items are removed or made inaccessible. Licensee agrees to submit a statement detailing the plan to be and remain in compliance. Written plan and pictures to be submitted to the Riverside Child Care Regional Office by 2/22/2023.
Based on observation, the licensee did not comply with the section cited above. In touring the backyard, LPA observed a spa/jacuzzi with a cover that was not latched or locked. Also, it was surrounded by a wrought iron fence that had sections with different heights, that did not meet Title 22 regulations. The door/gate did not swing away from the spa/jacuzzi nor did it self lock. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to make the backyard off limits, temporarily, drain the spa/jacuzzi, and place a lock in the gate. Licensee agrees to submit a statement detailing the plan to be and remain in compliance by 2/22/2023. Also, no later than 2/27/2023, the licensee agrees to be in compliance by removing, purchasing a functional cover, or installing the proper fencing with proof submitted to the Riverside Child Care Regional Office.
Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #5, did not have te required LIC 627 form (Consent for Emergency Medical Treatment) on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to submit a completed copy of the form to the Riverside Child Care Regional Office by 4/28/2022.
Based on record review, the licensee did not comply with the section cited above. The last fire and disaster drill was conducted over six month ago on 9/21/2021. This poses/posed a potential health, safety or personal rights risk to persons in care.
