Based on record review, the licensee did not comply with the section cited above. In reviewing facility documents, the last drill recorded was on 4/4/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill, log it, and submit a copy of the log to the Riverside Child Care Regional Office by 11/7/2025.

Based on observation and interview, the licensee did not comply with the section cited above. Upon arrival Licensee advised that they were closed with no children present and none scheduled to arrive however, about 5 minutes after the LPAs left the premises two children arrived and minutes after that a third child was dropped off as well. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written statement as to how they will ensure compliance with the cited regulation section. Statement to be submitted to the Riverside Child Care Regional Office by 11/7/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing administration files, the licensee and their assistant had not renewed their Mandated Reporter Training, as required. Last training was completed on 9/23/2023. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and submit proof of completion to the Riverside Child Care Regional Office by 11/7/2025.

Based on observation, the licensee did not comply with the section cited above. Upon arrival, LPA observed 10 children with only the licensee's assistant present providing care and supervision. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee arrived minutes later, during the inspection. Licensee agrees to submit a plan as to how she will assure compliance with regulation section cited. Plan to be submitted to the Riverside Child Care Regional Office by 3/28/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing the facility roster, there were/was child(ren) present that were/was not listed on the roster. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the facility roster and submit a copy to the Riverside Child Care Regional office by 4/3/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing files, Child #9, did not have an immunization record on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a copy of the child's immunization record and provided a copy to the Riverside Child Care Regional office by 4/3/2024.

Based on observation, the licensee did not comply with the section cited above. LPA toured the outside playground and observed dog feces on the yard/grass. At the time of the inspection, the children had not been oustide yet. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to clean up the back yard prior to allowing children to play outdoors. Licensee agrees to submit pictures of the outdoor playground and a plan on how to maitain the outdoor free from any items that could pose a danger to children in care. Pictures and plan to be submitted to the Riverside Child Care Regional office by 4/3/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #2, did not have the required form LIC 995A on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a signed copy of Parents Rights LIC 995A and submit a copy to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. The last drill that was logged took place in March 25, 2022. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a drill, log it, and submit a copy of the log to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. Both, the licensee and her assistant had expired (3/2022) CPR/1st aid cards. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to enroll in an EMSA approved CPR/1st aid course and submit proof of enrollment to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #2, did not have the required record on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain the required, Immunization Records, and submit a copy to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #2, did not have the required forms LIC 700 and LIC 627 on file.This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain completed/signed copies of forms LIC 700/LIC 627 and submit copies to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. The roster that was reviewed was not updated with dates of disenrollment and/or names of children currently in care. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to updated the facility roster LIC 9040 and submit a copy to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, Child #2, did not have the required form LIC 282 on file. This which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed/signed copy of form LIC 282 and submit a copy to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review and licensee's own admission, the licensee did not comply with the section cited above. Licensee had not yet began recording the 15 minute checks. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. Upon entering the facility, LPA observed child eating lunch, and in the facility refrigerator was only Vitamin D Whole milk. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase the milk that is required by the Health and Safety Code cited and submit proof of purchase to the Riverside Child Care Regional Office by 10/21/2022.

Based on observation, the licensee did not comply with the section cited above. Located in the bathroom that is assigned for the children to use, there were several items (mouthwash, toothpaste, hair products) with "KEEP OUT OF THE REACH OF CHILDREN" labels that were accessible to the children. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit pictures showing items have been made inaccessible and a written plan as to how compliance will be maintained. Both to be submitted to the Riverside Child Care Regional Office by 10/14/2022.

Based on record review and interview of licensee, the licensee did not comply with the section cited above. Present at the time of inspection were four children under the age of two and the licensee did not have an Infant Sleep log/chart, for any of them. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to compose an Infant Sleep Log/Chart to document the napping infant, every 15 minutes. The licensee agrees to submit copies of the logs to the Riverside Child Care Regional Office by 10/26/2021.

Based on observation of an infant (Child#1) sleeping in the garage, inside of a play yard, on their stomach, the licensee did not comply with the section cited above. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to review the Infant Safe Sleep regulations, that were provided to her upon initiating the inspection. Licensee agrees to submit a written statement regarding her understanding of the cited regulation and what the plan will be to assure compliance. Statement to be submitted to the Riverside Child Care Regional Office by 10/20/2021.

Based on record review for Child #1, the licensee did not comply with the section cited above. The required form was not available for review. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have the parent/guardian complete the form and then provide copies to the Riverside Child Care Regional Office by 10/26/2021.

Based on observation of an infant (Child#1) covered with a blanket and small toy attached to pacifier, the licensee did not comply with the section cited above. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The licensee removed the items during the inspection. Licensee agrees to review the Infant Safe Sleep regulations, that were provided to her upon initiating the inspection. Licensee agrees to submit a written statement regarding her understanding of the cited regulation and what the plan will be to assure compliance. Statement to be submitted to the Riverside Child Care Regional Office by 10/20/2021.

Based on record reviews for Child #1 and #2, the licensee did not comply with the section cited above. The form LIC 282 was not signed by the parent/guardian. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have the parent/guardian complete/sign the form and then provide copies to the Riverside Child Care Regional Office by 10/26/2021.

Based on record review of Child #3, the licensee did not comply with the section cited above. The child's file was missing the immunization records. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a copy of the required immunizations or documentation of contract termination. To the Riverside Child Care Regional Office by 10/26/2021.

Based on observation and interview of the licensee, she has made alterations and upgrades at the home, including the backyard, where the children play, and did not notify the Department. Therefore, the licensee did not comply with the section cited above. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: As of today's inspection, the licensee has placed the backyard "off limits" due to some hazardous conditions. The licensee agrees to submit a written statement/plan on how she will assure compliance with the cited regulation, along with dates of completion for all alterations being done at the facility.
