Based on observation, the licensee did not comply with the section cited above. In touring the facility LPA observed that the dial on the Fire Extinguisher was indicating that it was "empty". This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase a new fire extinguisher and submit proof of purchase to the Riverside Child Care Regional Office by 9/24/2025.

Based on interview, the licensee did not comply with the section cited above. Licensee stated that the milk served to children, including two year olds and above, is 2% or whole milk. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase milk (1%) and submit proof of purchase to the Riverside Child Care Regional Office by 9/30/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing facility records LPA observed that the Mandated Reporter Training certificate for their assistant had expired. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have their assistant renew the required training and submit proof of completion to the Riverside Child Care Regional Office by 9/30/2025.

Based on observation, the licensee did not comply with the section cited above. In touring the facility LPA observed Comet cleaner and loose plastic bags, in the cabinet underneath the kitchen sink. There were no locking mechanism or latch to prevent children from having access to those items. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the items during the inspection and agrees to submit a written plan as to how they will remain in compliance with the cited regulation section. Plan to be submitted to the Riverside Child Care Regional Office by 9/24/2025.

Based on record review, the licensee did not comply with the section cited above. The licensee discontinued logging the 15 minute checks, as required for infants. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to continue logging 15 minute check times and submit a copy of the log(s) to the Riverside Child Care Regional Office by 2/9/2024.

Based on record review, the licensee did not comply with the section cited above. In reviewing children records, it was discovered that Child #1 did not have the required Sleep Plan (LIC 9227) on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain a completed LIC 9227 and submit a copy to the Riverside Child Care Regional Office by 10/21/2022.

Based on record review, the licensee did not comply with the section cited above. LPA reviewed the Fire/Disaster drill log which showed the last drill being conducted in March 2022. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a drill, log it, and submit a copy of log to the Riverside Child Care Regional Office by 10/21/2022.

Based on observation, the licensee did not comply with the section cited above. LPA toured the back yard and observed the play structure with dirt and dust as previously observed during a visit conducted on 10/19/2021. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to clean/wash out all the dirt and dust off of the play structure prior to allowing children to play on it. Licensee also agrees to submit pictures, as proof, to the Riverside Child Care Regional Office by 10/21/2022.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed an adult (Dalila Reyes) at the facility. According to the licensee , they had been assisting at the facility on a few occasions within the last few weeks. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have staff live scanned as early as today (10/13/2022) and submit proof to the Riverside Child Care Regional Office by 10/14/2022. Along with that, licensee agrees to submit a plan on how she will remain in compliance with the cited regulation.

Based on record review, the licensee did not comply with the section cited above. The licensee and her assistant have not completed the required training. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the required training and have her assistant complete it as well. The licensee also agrees to submit proof of completion to the Riverside Child Care Office by 10/27/2021.
