Resolution: Licensee agrees to complete the required training and submit proof of completion to the Riverside Child Care Regional Office by 4/30/2024.
Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #2 had an incomplete/unsigned LIC 282 form on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have parent/legal guardian complete the required form and licensee agrees to submit a copy to the Riverside Child Care Regional Office by 4/30/2024.
