ALVAREZ FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above. In reviewing children file, it was determined that child #1 and #2, did not have updated immunization records on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain updated documentation regarding the children's immunization record and submit copies to the Riverside Child Care Regional Office by 3/31/2025.

Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #5, did not have a signed form (LIC 627) on file, as required. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have the child's parent/guardian sign the required form (LIC 627) and submit a copy to the Riverside Child Care Regional Office by 6/7/2022.

Based on record review and licensee's own admission, the licensee did not comply with the section cited above. The facility roster did not have all names and information of children present. This poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to update the facility roster and submit a copy to the Riverside Child Care Regional Office by 6/7/2022.

Based on observation and licensee's own admission, the licensee did not comply with the section cited above. The licensee provides whole milk or 2%, also the juice provided is only 10% juice. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to serve the required milk and/or juice. Licensee agrees to purchase the appropriate milk/juice and provide pictures of the items and of the purchase receipt, as proof of purchase. Pictures to be submitted to the Riverside Child Care Regional Office by 5/25/2022.
