Based on observation, the licensee did not comply with the section cited above due to the licensee being alone with 12 preschool age children in care which poses an immediate health, safety or personal rights risk to persons in care. Per licensee, her assistant was away from the facility for approximatly 30 minutes due to picking up her son from school.
Resolution: Deficiency was corrected when the Licensee's assistant returned back to the facility. Licensee had 12 children alone when LPA Plumboy entered the facility for an inspection. LPA Plumboy reviewed licensees comments on her license, the ratio guidelines, and previous reports with licensee.
Based on observation, the licensee did not comply with the section due to the fence dividing her yard with her neighbors yard located on the right side of her home falling over which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: On or before 4/13/24, licensee will send an estimate/quotes stating how much it will cost to fix her fence as well as what her plan is to have the fence repaired.
Based on record review, the licensee did not comply with the section cited above due to her assistant present today not having current immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: On or before 3/15/23, licensee will provide LPA Plumboy with the immunizations for her assistant. The immunizations include MMR, TDAP, and INFLUENZA (the influneza may be declined with a wriiten statement). Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.
Based on record review, the licensee did not comply with the section cited above in 1 out of 4 files do not have an immunization record which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: On or before 2/24/23 licensee shall transfer C1 immunizations onto immunization cards and submit copies to LPA.
