Based on observation on 8/27/25, no daily log for pool checks, the licensee did not comply with the section cited above in [1] out of [1] daily pool check log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: On visit date 10/27/25 Licensee had a log check for pool daily inspection, POC cleared during visit 10/27/25.

Based on observation on 8/27/25, visit bottom of the pool door vertical clearance measured at 2 3/4 inches, the licensee did not comply with the section cited above in [1] out of [1]which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Mesh fence will be observed and bottom of the door passage will be fixed or replaced to measure a max. of 2 inches.

Based on record review no proof that mesh fencing is in compliant with the American Society for Testing and Materials (ASTM) International Standard F2286. Documentation on 8/27/25 ASTM report shows no correlation that the report is specific to the address home above, the licensee did not comply with the section cited above in [1] out of [1] no proof of mesh fence standard for the specific address of day care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Proof of mesh fence meeting American Society for Testing and Materials (ASTM) International Standard F2286 for the day care, to send document to LPA Calderon by poc due date.

Based on record review conducted by LPA Calderon no infant 15 min sleep log, the licensee did not comply with the section cited above in [1] out of [1] 15 min sleep log/ infant document requirement which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA Calderon gave 15 min log sample that they can use or create a new one. 15 min log for Child #1 (infant) will be submitted to LPA Calderon by poc due date.

Based on LPA Calderon's observation, LPA observed Licensee using the Fence latch knob to pull it up to open the pool. Per Licensee, a key is needed to lock pool, no key was observed when Licensee opened the door and per record review of the pool documentation's provided to LPA Calderon on instructions on how to securely lock pool door a key is required to lock door, the licensee did not comply with the section cited above in [1] out of [1] pool safety requirement does not meet regulations and stature, door can be opened using pulling knob which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: During visit LPA observed Licensee locked the pool to provide inaccessbility. POC cleared during visit.

Based on interview Per Licensee, stated downstairs den/ bedroom was on limit to use as a walkway to the backyard but not in use anymore and it is off limits. Per Licensee, the facility goes through the garage to go access the back yard. Per Licensee, sometimes children us the back yard to play. Per review of the Facility Sketch LIC999 Backyard states off limit; changes of the back yard will be submitted to the department, deficiency cited. Back yard has a fenced pool. , the licensee did not comply with the section cited above in [2] out of [2] changes to the facility physical plant not reported to the department which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Facility Sketch LIC999 for inside physical plant and back yard will be submitted to LPA Calderon by poc due date to LPA Calderon's email.

Based on LPA Calderon observation At 9:38am (3) Poison bottles of insect killer spray bottles were observed in cabinet above the kitchen stove, licensee moved poison bottles to the garage. LPA observed Licensee lock screen door that locks with a key lock, the licensee did not comply with the section cited above in [3] out of [3] accessible poisons to children in care which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Correction fixed during visit ,licensee moved poison bottles to the garage. LPA observed Licensee locked garage screen door that locks with a key lock to enter the garage. Poisons were locked. Cleared during visit.

Based on observation, interview and record review, Licensee did not renew the mandated reporter training, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete the training and licensee will email the certification to LPA's email by 05/27/2024

Based on observation, interview, record review, the licensee did not comply with the section cited above. LPAs observed an unfingerprinted adult. Licensee stated that assistant Monqiue Wallace is not fingerprinted. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant understand that she cannot work at the facility until fingerprints are cleared. Assistant immediately left facility.
