Based on LPA Calderon observation At 2:04pm, there was Isopropyl Alcohol 70% first aid antiseptic in the bathtub of the hallway restroom that the children use and in addition, LPA Calderon observed 3% hydrogen peroxide tropical solution on the ledge of the bathtub, Per Licensee, children do not use restroom alone, picture taken, the licensee did not comply with the section cited above in [2] out of [2] irst aid liquids/products were not placed in an inaccessible area which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed items and placed items in a high to reach shelf located in the kitchen. Cleared during visit.

Based on LPA Calderon observed (3) play yards to have a soft mattress and no fitted sheets, picture taken at 2:03pm and LPA observed 4 infants in care and (2) bassinets, the licensee did not comply with the section cited above in [3] out of [3] play yards no not have proper mattress and fitted sheets which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee Taylor will purchase firm mattress' and fitted sheets for (4) play yards and bassinets were removed during visit and put in off limit areas, pictures will be sent to LPA Calderon.

Based on observation, the licensee did not comply with the section cited above in [1] out of [1] objects - fire extinguisher was purchased 7/17/23 and was not current within 1 year of purchase, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will get extinguisher services or purchase and show proof of purchase. Licensee will send LPA image of proof by poc due date.

Based on observation, the licensee did not comply with the section cited above in [2] out of [2] objects , attached pacifiers were placed on 2 persons (infants Child #3 and Child #4) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA gave PIN 20-24-CCP & infant safe sleep regulation 102425 to review. Licensee will write declaration statement on reviewing infant safe sleep regulation and pin and state (3) things they learned as part of training and have staff sign as well. Licensee will send documentation's to LPA Calderon by poc due date.
