Based on LPA observation Fire extinguisher was observed 3:04pm with a service tag with not date of inspection and LPA observed no screen on wall heater, pictures taken for both items, the licensee did not comply with the section cited above in [1] out of [2] 2 items fire ext. and wall heater does not meeting standard regulations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee Wall Heater screen barricaid will be added and fire extinguisher will be serviced, pictures or visit will be conducted for proof of corrections.

Based on LPA Calderon observation Infant play yard observed, LPA observed 1 infant play yard not sleeping, there was a fitted sheet that was around the thin pad that Per Licensee comes with the play yard and no firm mattress observed, pictures taken, the licensee did not comply with the section cited above in [1] out of [1] infant sleeping mattress which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase firm mattress for play yard and send picture or visit will be conducted.

Based on LPA Calderon reviewed Aug-Sept 15 min sleep log for Child #1, infant was present during visit at 4:00pm Licensee informed LPA needs to find it but will email it to LPA, the licensee did not comply with the section cited above in [1] out of [1] infant 15 min check log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee: infant 15 min check log will be provided toLPA Calderon by poc due date.

Based on LPA Calderon observation of infant Child #1, was sleeping on a 'Bobbi Pillow' on the couch elevated and not sleeping flat on their back Per interview with Licensee, stated Child#1 has medical issues, LPA requested exemption documentation's, per Licensee unable to provide documentation's and needs to collect document from child's parent. , the licensee did not comply with the section cited above in [1] out of [1] persons which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee during time of visit immediately transferred Child #1 to a play yard. Licensee stated will speak to Child #1 mom who brings infant items for child. LPA provided Infant Safe Sleep Regulations 102425 and Pin 10-2024 to review.

Based on observation LPA did not observe a carbon monoxide device and per Licensee needs to get a new one as device broke, the licensee did not comply with the section cited above in [1] out of [1] objec] which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a carbon monoxide and show proof of device and recipet to LPA Calderon by POC due date.

Based on observation LPA Calderon did not observe fire extenguisher service inspection tag or was not provided receipt of proof of purchase within 1 year, the licensee did not comply with the section cited above in [1] out of [1] objects, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase a new fire extinguisher, or show proof of serviced inspection tag LPA Calderon by POC due date.

Based on observation LPA Calderon observed poison Spectacide Bug Killer under kitchen sink, the licensee did not comply with the section cited above in [1] out of [1] item which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed poison and stored in locked garage.

Based on LPA observed infant rocker no child in rocker, Licensee stated rocker brought to the facility by Child #1 parent , the licensee did not comply with the section cited above in [1] out of [1] persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will return infant rocker to Child #1, LPA regulations on prohibited items. LPA during visit placed rocker in off limit area.

Based on observation LPA Calderon did not see any fitted sheets avaiable for Child #1, the licensee did not comply with the section cited above in [1] out of [1] persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated will purchase sheets for infants play yards/ cribs and in addition will ask parents to provide sheets, Licensee will provide pictures to LPA Calderon via mobile phone by POC due date.

Based on observation Licensee did not provide LPA Calderon during record review Mandated Reporter Training Certificate, the licensee did not comply with the section cited above in [1] out of [1] person which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct Child Care Provider Mandated Reporter training, and will provide picture of certificate to LPA Calderon via mobile phone by poc due date.

Based on observation LPA Calderon observed expired CPR and First Aid , expired on 7/2023, the licensee did not comply with the section cited above in [1] out of [1] person which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct Pediatric First Aid and CPR training and will send certificate by POC due date to LPA Calderon mobile phone.

Based on observation and record review LPA Calderon did not observe children's records for Child #1, Child #4, Child #5 and Child #6, per Licensee unable to locate, the licensee did not comply with the section cited above in [4] out of [6] persons which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will locate and gather records for Child #1, Child #4, Child #5 and Child #6, Licensee will send pictures of records to LPA Calderon by POC due date. LPA Calderon provided verbally list of records: LIC311D for licensee to use as guidance and LPA Calderon provided Entrance FCC checklist.

Based on LPA Calderon observation during record review Licensee was unable to provide 15 min sleep log for Child #1, the licensee did not comply with the section cited above in [1] out of [1] persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide 15 min sleep log of Child #1 sleep log by POC due date to LPA Calderon. LPA Calderon provided 15 min Sleep Log Sample for Licensee to use if they would like.

Based on observation LPA observed outside back yard play area, LPA observed piles of high stacked storage containers approximately 3-5 ft, pile of recycled cans in plastic bags, motor vehicle bikes in backyard grass area, leaf blower and lawn mower. The licensee did not comply with the section cited above in [5] out of [5] objects which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will rerrange back yard items that pose a danger to children in care and store in off limit areas, will add baby gates to the side runners of the home to store items. Pictures of backyard will be sent to LPA Calderon by POC due date.

Based on observation LPA Calderon observed Licensee placing a light loose sheet for Child #1 for child to sleep on, Per Licensee does not have fitted sheets for play yard, the licensee did not comply with the section cited above in [1] out of [1] person which posesa potential health, safety or personal rights risk to persons in care.

Resolution: Licensee during visit removed light loose sheet from crib, LPA informed Licensee no loose items in cribs and play yards for infant safe sleep practice and only use a fitted sheet.
