Resolution: Per Licensee Wall Heater screen barricaid will be added and fire extinguisher will be serviced, pictures or visit will be conducted for proof of corrections.

Based on LPA Calderon observation Infant play yard observed, LPA observed 1 infant play yard not sleeping, there was a fitted sheet that was around the thin pad that Per Licensee comes with the play yard and no firm mattress observed, pictures taken, the licensee did not comply with the section cited above in [1] out of [1] infant sleeping mattress which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will purchase firm mattress for play yard and send picture or visit will be conducted.

Based on LPA Calderon observation of infant Child #1, was sleeping on a 'Bobbi Pillow' on the couch elevated and not sleeping flat on their back Per interview with Licensee, stated Child#1 has medical issues, LPA requested exemption documentation's, per Licensee unable to provide documentation's and needs to collect document from child's parent. , the licensee did not comply with the section cited above in [1] out of [1] persons which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee during time of visit immediately transferred Child #1 to a play yard. Licensee stated will speak to Child #1 mom who brings infant items for child. LPA provided Infant Safe Sleep Regulations 102425 and Pin 10-2024 to review.

Resolution: Licensee will locate and gather records for Child #1, Child #4, Child #5 and Child #6, Licensee will send pictures of records to LPA Calderon by POC due date. LPA Calderon provided verbally list of records: LIC311D for licensee to use as guidance and LPA Calderon provided Entrance FCC checklist.

Resolution: Licensee will provide 15 min sleep log of Child #1 sleep log by POC due date to LPA Calderon. LPA Calderon provided 15 min Sleep Log Sample for Licensee to use if they would like.

Based on observation LPA observed outside back yard play area, LPA observed piles of high stacked storage containers approximately 3-5 ft, pile of recycled cans in plastic bags, motor vehicle bikes in backyard grass area, leaf blower and lawn mower. The licensee did not comply with the section cited above in [5] out of [5] objects which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will rerrange back yard items that pose a danger to children in care and store in off limit areas, will add baby gates to the side runners of the home to store items. Pictures of backyard will be sent to LPA Calderon by POC due date.

Based on observation LPA Calderon observed Licensee placing a light loose sheet for Child #1 for child to sleep on, Per Licensee does not have fitted sheets for play yard, the licensee did not comply with the section cited above in [1] out of [1] person which posesa potential health, safety or personal rights risk to persons in care.

Resolution: Licensee during visit removed light loose sheet from crib, LPA informed Licensee no loose items in cribs and play yards for infant safe sleep practice and only use a fitted sheet.
