Based on record review staff #1 Immunization records missing TDAP and MMR and Staff #2 missing in records MMR, the licensee did not comply with the section cited above in [1] out of [3] Staff immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Immunization records will be retrieved or staff will get immunization shot / reading done and send records to LPA Calderon by poc due date.

Based on record review Infant #1 and Infant #2 15 min logs were inconsistent, did not have time of 15 min checks logged on document, the licensee did not comply with the section cited above in [2] out of [2] infant 15 min sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: 15 min sleep log will be sent to LPA Calderon by poc due date for infants #1 and #2.

Based on observation, the licensee did not comply with the section cited above in [1] out of [1] closed door where infants are sleeping which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Regulation Infant Safe Sleep 102425 and Licensee will review regulations and write 3 things learned and write it on LIC855 Declaration Letter and send to LPA by poc due date.

Based on record review Licensee Mandated Reporter expired 6/2/25, Staff Assistant #1 (S1) Mandated Reporter Training was not provided, Staff #2 (S2) mandated reporter training expired 6/9/2024; picture of certificate , the licensee did not comply with the section cited above in [3] out of [3] mandated certificate not valid which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Staff #1, Staff #2 and Licensee will conduct 4 hour Mandated Reporter- Child Care Provider and sent certificates to LPA Calderon by poc due date.

Based on record review Pediatric First Aid and CPR for Licensee, Staff #1 and Staff #2 expired 2/27/25, picture taken of certificates; the licensee did not comply with the section cited above in [3] out of [3] expired Pediatric First Aid and CPR which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Pediatric First Aid and CPR will be conducted. During visit Licensee scheduled a class for Licensee and Staff Assistants. Certficates will be sent to LPA Calderon by poc due date.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Per Licensee one child is scheduled to leave at 2pm today. Licensee will write a plan on ensuring capacity will not be over limit of 14 children at one time, declaration letter will be sent to LPA Calderon by poc due date.

Based on observation Infant #1 had an attachment on pacifer and their clothing while sleeping in crib in the nurserey, the licensee did not comply with the section cited above in [1] out of [2] infants sleeping with attachment which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Regulation Infant Safe Sleep 102425 and Licensee will review regulations and write 3 things learned and write it on LIC855 Declaration Letter adn send to LPA by poc due date.

Based on observation there were 3 colorful plastic rings hanging on the ledge of crib where infant #1 was sleeping , the licensee did not comply with the section cited above in [1] out of [1] hanging objects on crib which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA provided Regulation Infant Safe Sleep 102425 and Licensee will review regulations and write 3 things learned and write it on LIC855 Declaration Letter adn send to LPA by poc due date.

Based on observation and interview, the licensee did not comply with the section cited above in fire extinguisher not being serviced since August 2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit picture proof of serviced fire extinguisher via email to assigned LPA.

Based on record review, the licensee did not comply with the section cited above due to Staff #1 did not have the required immunizations on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, Staff will no longer work at facility. The last day is 1/14/22. Licensee will ensure that all new emplyees have they immunizations prior to stating emplyment, A written staemnet was obrained from licensee.

Based on record review, the licensee did not comply with the section cited above in 1 out of 6 files were not available. Child #2 was missing file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, a copy of Child #2's facility file will be sent to department via email py POC due date. Licensee has provided a written declaration stating they will insure all children receiving care at the above faciltiy will have a complete faciltiy file prior to attending facility.

Based on record review, the licensee did not comply with the section cited above based on Child #1, an infant under 12 months of age was missing the LIC 9227 in the file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, a completed LIC 9227 Infant Sleep Plan will be sent to the Department via email by POC due date.
