Resolution: The Licensee and staff will complete the Child Care Provider Mandated Reporter Training, and submit a copy of the training certificates to the LPA by the POC date of 4/9/23.

Resolution: Licensee will provide proof of immunization against influenza, pertussis, and measles for themselves and staff by the POC date of 5/13/22. Licensee and staff can also submit a declination letter if they choose not to take the influenza vaccine.
