Based on LPA Calderon observed at 9:40am fire extinguisher with a receipt dated 9/30/2024, date purchase was observed not to met State Fire Marshall Regulations and was not within 1 year, picture taken, the licensee did not comply with the section cited above in [1] out of [1] fire extinguisher that does comply with State Fire Marshall Regulations which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Picture of fire extinguisher with a receipt or service tag within 1 year will be sent to LPA Calderon by poc due date.
Based on record review conducted by LPA Calderon infant Child #1 present during visit, the child is in the age range of 0mth- 12mth's and Licensee was unable to provide LIC9227 Infant Indiv. Safe Sleep Plan for infant, the licensee did not comply with the section cited above in [1] out of [1] missing document for Infant which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide parent with LIC9227 Infant Indiv. Safe Sleep Plan and a copy will be sent to LPA Calderon by poc due date.
