Based on LPA Calderon observation fire extinguisher was had an service tag dated 4/10/2021, no inspected within 1 year, the licensee did not comply with the section cited above in [1] out of [1] fire extinguisher \ which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide fire extinguisher with updated service tag inspected within 1 year, and send picture to LPA Calderon via email by poc due date.
Based on record review Licensee and Staff #3 had cpr and first aid that did not have an EMSA sticker or from a approved vendor, the licensee did not comply with the section cited above in [2] out of [3] staff pediatric cpr/first aid records, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide Pediatric first aid and cpr certifications with EMSA sticker, from Red Cross or American Heart Association for Licensee and Staff #3 via email to LPA Calderon by poc due date.
Based on record review Staff #3 did not have immunization from Measles and Tuberculous, the licensee did not comply with the section cited above in [1] out of [3] staff records which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain TB/MMR immunization records from Staff #2 and provide copy to LPA Calderon via email by poc due date.
