All children are under supervision, including visual supervision, of a teacher at all times. Facility meets teacher-child ratios. LPA reviewed a sample of children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed a sample of staff files and observed files were mostly complete with health screening, immunization records for influenza, pertussis and measles and current documentation of completed mandated reporter training; one staff was missing the mandated reporter training as well as her immunization records. LPA discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to resources such as forms, regulations Provider Information Notices (PINs), and Quarterly Updates. LPA discussed Reporting Requirements as outlined in the regulations (Section 101212). Ms. Puga was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPAs observed that a teachers aid was not on the staff roster of cleared adults, LPAs verified that staff was finger printed at the time of hire but failed to complete a LIC 508 so the finger prints were never associated to the facility. Based on observation and interview, the licensee did not comply with the section cited above. LPA observed required cushioning under climbing structure to absorb a fall. LPA observed a wooden walking beam in the play yard. Wooden bean was half covered with fabric material but the fabric is falling off exposing the wood and its not protected. This poses a potential health, safety or personal rights risk to persons in care.
Based on observation, interview and record review, the licensee did not comply with the section cited above. During staff file review, 3 out of 6 staff were missing immunizations. This poses a potential health, safety or personal rights risk to persons in care.
Based on observation LPA's observed outdoor wooden bean having fabric material ripped apart, hanging off bean, per SAVITRI YASODA ABEYAWARDEN stated soemtimes children use it, the licensee did not comply with the section cited above in [1] out of [1] outdoor wooden bean object that can cause a fall, which poses a potential health, safety or personal rights risk to persons in care.
Personnel Requirements. All personnel including the licensee shall have a health-screening report, including specified information, signed by the person who performed it. During this inspection this requirement is not met by evidence, Staff #1,2,3, 5, and 6 did not health screening report. This poses a potential risk to the health and safety of the children in care.
