Resolution: Licensee will submit infant sleep plan completed by parents to LPA by POC due date.
Based on observation the licensee did not comply with the section cited above in there are rose bushes with protruding thorns in a flower bed in the outdoor playyard where children play with no visible barrier, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide of inaccessible rose bushes and submit to LPA by POC due date.
Based on record review the licensee did not comply with the section cited above in 2/2 Staff Who did not have Valid Pediatric First Aid CPR certification. Certification on file was labled as K-12 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of appointment for Pediatric First Aid CPR Certificatiion for both licensee's by POC due date to LPA.
Based on record review, the licensee did not comply with the section cited above in 1 out of 3 staff did not have Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit Current proof of Mandated Reporter training to LPA by POC due date.
Based on observation and record review, the Licensees did not comply with the section cited above in 1 out of 1 fire extinguishers which poses a potential health, safety or personal rights risk to persons in care. The fire extinguisher has not been serviced in over a year.
Resolution: Licensees will purchase a new fire extinguisher and send a copy of the receipt to LPA by the POC date of 9/29/22.
