Based on record review Staff Assistant did not have mandated reporter training in file, Licensee did not provide certificate for Staff assistant, the licensee did not comply with the section cited above in [1] out of [1] staff assistant Mandated Reporter -Child Care Provider which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Staff assistant will take Mandated Reporter -Child Care Provider (4 hr) training and send certficate to LPA Calderon by poc due date.

Based on LPA Calderon record review staff assistant did not have immunization records - TDap, MMR, TB and Influenza, the licensee did not comply with the section cited above in [1] out of [2] staff immunization records which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Staff assistant will obtain immunization records for- TDap, MMR, TB and Influenza (can decline flu) or will get immunization's done if needed and send copies of records to LPA Calderon.

Based on record review Licensee Pediatric First aid and CPR expired in June of 2025, picture taken of certficate, the licensee did not comply with the section cited above in [1] out of [1] Licensee Pediatric First aid and CPR expired which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will take Pediatric First aid and CPR and send a copy to LPA Calderon by poc due date.

Based on observation and interview, the licensee did not comply with the section cited above. LPAs observed fire extingusiher was last serviced on 10/17/2022. Licensee stated she didn't realized it wasn't serviced in 2023, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee will get fire extinguisher will be serviced by POC due date of 01/25/24 and will submit photos to LPA via email.

Based on observation, interview, and record review)], the licensee did not comply with the section cited above. During record review, LPAs did not observe a current Mandated Reporter training. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee will submit an updated Mandated Reporter by POC due date 02/02/24 via email.

Based on interview and record review, the licensee did not comply with the section cited above. Child #4 & #5 were missing (LIC) 282- Affidavit Regarding Liability Insurance, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights LIC 9227 LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submitt a photo by POC due date of 2/02/24 via email.
