Based on observation at 1:50pm baby walker was observed in living room, on limit area to the children, per interview with Licensee states sits infant #1 in baby walker but does not use it with the wheels, picture was taken, the licensee did not comply with the section cited above in [1] out of [1] baby walker which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee during visit, placed baby walker in off limit bedroom and understands cannot use baby walker for day care children.
Based on observation LPA Calderon and LPA Tyler Reyes did a record review for Infant #1, Licensee unable to provider LIC(227 Indiv. Infant Safe Sleep plan, during interview stated not familiar with form, the licensee did not comply with the section cited above in [1] out of [1] form for infant #1 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA Calderon Provided LIC9227 Indiv, Safe Sleep Plan, Licensee will have parents of Infant #1 fill out form and will send copy to LPA Calderon via email by poc due date.
