Based on LPA Calderon's observation during record review Licensee and staff assistant mandated reporter expired 1/21/24 , the licensee did not comply with the section cited above in [2] out of [2] expired mandated reporter certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will take mandated reporter training and certificate will be sent to LPA Calderon by poc due date.
Based on LPA Calderon's observation during record review Licensee was unable to provide 15 min sleep log for child #1 and child #2 (both infants), the licensee did not comply with the section cited above in [2] out of [2] infants in care which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided 15 min sleep log sample to Licensee, Licensee will use log and log 15 min time for infants #1 and #2 and send to LPA Calderon by poc due date.
