Based on LPA Calderon record review observed certficate of pediatric first aid and cpr expired 5/9/25, the licensee did not comply with the section cited above in [1] out of [1] pediatric first aid and cpr which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit Pediatric 1st aid and cpr to LPA Calderon by poc due date.
Based on observation, interview, record review, the licensee did not comply with the section cited above. During staff file review, LPA did not observe any immunization record for Licensee. Licensee stated that she did not have her immunization record. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee states that she will submit a copy of her immunization record via email by POC due date of 06/05/23.
