Based on record review Staff Assistant ( Staff #2) file did not have Mandated reporter training, Licensee stated Staff #3 certficate expired, the licensee did not comply with the section cited above in [1] out of [2] Staff assistant mandated reporter which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Staff Assistant ( Staff #2) will take Child Care Provider Mandated Reporter Training and will send certificate to LPA Calderon .
Based on record review and interview the licensee did not comply with the section cited above: Licensee could not provide current Mandated Reporter which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of current Mandated reporter training via email by POC due date of 10/26/23.
Based on observation, the licensee did not comply with the section cited above in 1 out of 1 uncleareed individuals (Desirai Lewis) present in the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will have daughter Desirai Lewis re- fingerprinted today and will not allow her be present at the daycare during hours of operation until she has obtained a criminal record clearance. Licensee stated she would provide s copy of the completed livescan and will email a copy to LPA 08/31/2022.
