Based on observation on 8/20/25 fire extinguisher service tag dated on 07/22/24 , the licensee did not comply with the section cited above in [1] out of [1] fire extinguisher not comply State Fire Marshall regulations which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Fire extinguisher Service tag observed on 9/12/25 dated Aug 21, 2025. POC cleared on today's date.
Based on record review Licensee Pediatric 1st aid and cpr certificate issued by National CPR Foundation certficfate observed with no EMSA Stickers and certificate staff assistant did not have a CPR/1st aid certificate, the licensee did not comply with the section cited above in [2] out of [2] staff did not have a valid Pediatric 1st aid and cpr certficate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Staff Assistant (Licensee's husband) will obtain a valid Pediatric 1st aid / cpr certficiate and submit to LPA Calderon by poc due date.
Based on record review on 8/20/25 Child #1 and Child #2 15 min sleep log showed start of sleep time and end of sleep time, it was observed not to show time of 15 min checks, the licensee did not comply with the section cited above in [2] out of [2] incomplete (2) infant records which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit for Child #2, 15 min sleep log showing 15 min time frame to LPA Calderon by poc due date.
Based on 9/11/25 interview via telephone with Licensee Marisol Andrews informed LPA Calderon UPS did not have livescan form and on visit date 9/12/25 based on record review Live Scan form was provided to LPA Calderon Section #8 Live Scan Transaction Completed By observed blank, Per Licensee Marisol Andrews went on 9/11/25 to UPS office and they stated fingerprinted system is down. On 8/20/25 facility was cited for a resident who was not fingerprinted cleared, poc was to do livescan form and submit form to LPA Calderon by 8/21/25 live scan not submitted and observed incomplete on 9/12/25 the licensee did not comply with the section cited above in [1] out of [1] non fingerprint cleared adult resident living in the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will get Adult Son live scanned, Per Licensee will attempt to go to several UPS stores to get Adult Son fingerprinted, LPA Calderon provided number of live scan reservation 1-800-315-4507 and Licensee will submit Live Scan form to LPA Calderon by poc due date.
Based on record review LPA Calderon, the licensee did not comply with the section cited above in [1] out of [1] staff did not have Mandated Reporter Certfication which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee Husband staff assistant will conduct training online for Mandated Reporter and will send a copy to LPA Calderon.
Based on interview per Licensee M.Andrews uses the same play pen and sleeps in play pen for infants. LPA observed two play pens and not play yards or cribs with firm mattress and fitted sheets , the licensee did not comply with the section cited above in [2] out of [2] infant sleeping equipment which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will send pictures of purchased play yards or cribs with firm mattress and fitted sheets by poc due date.
Based on interview with Licensee M.Andrews informed LPA son is 19 years old.LPA asked Licensee was son fingerprinted cleared when he turned 18 years old, Licensee no not yet, son still lives at home, the licensee did not comply with the section cited above in [1] out of [1] adult not fingerprinted cleared which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will set up appoitnment for Live scan, appointment for TB (current) with MD, LIC279 application and Lic289B current children living in the home updated and sent to LPA Calderon and to the department by poc due date. Once live scan is processed and clearance letter is received, Licensee will submit forms to LPA Calderon via email.
Based on observation wine bottles on wine bar low to ground in kitchen, and low detergants in laundry room has baby gate and door knob has no lock, and poison in high shelve above laundry machines, poison thrown away in city trash can outside of home, the licensee did not comply with the section cited above in [1] out of [3] [(objects) items which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Lock o laundry room door, wines duirng visit moved to off limit upstairs area, will send picture of lock and new area where wines will be stored to LPA Calderon by poc due date.
Based onobservation and interview with licensee, the licensee did not comply with the section cited above. LPA observed a infant sleeping half in the sofa. The infants upper body was on the sofa and the rest of the body was standing. LPA observe a third infant sleeping in the high chair. Licensee stated that another infant usually sleeps in the bouncer which poses an immediate health, safety and personal rights risk to persons in care.
Resolution: Per Licensee, will start using a play yard and the other infants will sleep in cots by 11/02/22 Licensee will review the safe sleep regulations and write a summary and submit to LPA by 11/10/22.
Based on observation, interview and record review, the licensee did not comply with the section cited above. There were 4 infants upon arrival and 4 children which poses an immediate safety and personal rights risk to persons in care.
Resolution: Per Licensee, will make arrangments with parents so Licensee is not over ratio until Licensee is grant a license for a large.
Based on observation and interview, the licensee did not comply with the section cited above. Per Licensee each child has their own sleeping cots and a play yard, however they children fell asleep before taking out the cots. Per Licensee, she will start using a play yard for the younger infant instead of the bouncer which poses a potential health, safety and personal rights risk to persons in care.
Resolution: Per Licensee, once children are falling asleep, she will put children in their cots. Licensee also stated she will develop a plan with legal guardian of infant #4 to arrange child to sleep in a play yard.
Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee stated she has not been documenting the 15 minutes sleep check which poses a potential health, safety and personal rights risk to persons in care.
Resolution: Per Licensee will start documenting children sleeping and summit a copy to LPA by due date of 11/15/22 via email.
