Based on observation and interview, the licensee did not comply with the section above, in that her adult daughter,Andrian Eagleston, did not obtain a criminal record clearance, prior to watching the children in care.
Resolution: Andrian Eagleston left the facility at apprxomatley 4:30PM on 4/2/2025. Licensee was unable to state plan of correction, as she was traveling out of state. Assistant, Sharlean Peck, states that she will provide documents to the Licensee, and Licensee will reach out to LPA directly.
Based on observation and interview, the licensee did not comply with the section above, because there were 12 children present with Licensee's daughter, Andrian Eagleston, and no additional staff present.
Resolution: Assistant, Sharlean Peck and Sadie Holdaway, arrived at the facility around 3PM 4/2/2025. The facility was then in ratio. Licensee was unable to state plan of correction, as she was traveling out of state. Assistant, Sharlean Peck, states that she will provide documents to the Licensee, and Licensee will reach out to LPA directly.
Based on observation and interview, the licensee did not comply with the section above, in that the Licensee explained to the LPA via phone that she was out of state due to a family emergency, that will is longer than 20 percent of hours
Resolution: Licensee has not be present at the facility since 3/31/25 per the Licensee's Assistant, Sharlean Peck. Licensee was unable to state plan of correction, as she was traveling out of state. Assistant, Sharlean Peck, states that she will provide documents to the Licensee, and Licensee will reach out to LPA directly.
