LPA met with Director, Barbara Radi and toured the facility. Also present in the facility were 38 daycare children and 9 teachers/staff. Facility was within ratio & capacity. Staff members have the required Criminal record clearance and are associated to the facility. LPA interviewed staff, examined the drinking fountain, and inspected the fixture deemed an Action Level Exceedance. Fixture reported with 5.5 ppb or greater lead exceedance levels was as follows: G” Drinking Fountain located in Room 2 - 8.30 ppb Licensee reported that drinking fountain "G" valve was shut off to cease the use of the drinking fountain. The drinking fountain was shut off May 2020. The school provides filtered water and safe water from the passing faucets for refills to the children's personal water bottles. There is a faucet in the kitchen used for food prep that passed testing. An email letter was sent out to parents regarding the testing and remediation on 1/23/23. LPA observed that the water testing results are posted in the facility. See LIC809-D for cited deficiency. Exit interview was conducted and report was reviewed with the Director, Barbara Radi. A copy of Appeal Rights and Notice of Site Visit was given and must remain posted for 30 days.
