Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C3, C4, C5, and C9 were missing proof of the 15 minute sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document the 15 minute checks for all infants enrolled and agrees to begin document the 15 minute checks for C3, C4, C5, and C9. Licensee agrees to submit proof of the Plan of Correction (POC) by the end of the business day on the POC due date of 04/23/2024.

Based on observation and interview, the licensee did not comply with the section cited above as LPAs observed and were told that children were being served 2% milk which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to stop providing 2% milk to children effective immediatley and agrees to write a plan of action detailing Licensee's plan to ensure compliance with the above regulation. Licensee agrees to submit proof of the Plan of Correction (POC) by the end of the business day on the POC due date of 04/23/2024.

Based on observation, the licensee did not comply with the section cited above as there is a pool present at the facility with fencing that does not prevent direct access to the pool as two windows lead directly from the facility to the backyard pool area which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action detailing how the area with windows will be kept off-limits at all times to daycare children. Licensee agrees to correct the pool fence to meet regulation requirements within 30 days. Licensee agrees to submit proof of the Plan of Correction (POC) by the end of the business day on the POC due date of 04/23/2024.

Based on record review, the licensee did not comply with the section cited above as LPA observed that C1 and C8 were missing proof of immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C8's authorized representatives submit proof of immunizaitions. Licensee agrees to submit proof of the Plan of Correction (POC) by the end of the business day on the POC due date of 04/23/2024.
