Based on observation, interview and record review, the licensee did not comply with the section cited above Pediatric First Aid and CPR expired December 2025 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will renew the Pediatric First Aid/CPR or EMSA approved training and submit a copy to the Palmdale Regional office by due date specified by email.
Based on observation and interview, the licensee did not comply with the section cited above. LPA observed the gate self-latch and self-close devid is not working, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee agree to fix the self-latch and self-close devid. After complete it, the licensee will text LPA to recheck.

Resolution: The licensee and his wife will complete the training and email the certifications to the LPA.
