Based on record review, the licensee did not comply with the section cited above in Licensee did not ensure staff #1 had an updated Mandated Reporter Certificate, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will send the department an email with a copy of the completed mandated reporter training certificate for staff #1 by 9/29/23.
Based on record review, the licensee did not comply with the section cited above in Licensee did not have a current First Aid & CPR and expired on 8/14/23, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will send the deparment an email with a copy of a current First Aid & CPR by 9/29/23.
