Based on observation and Licensee's statement she stated she has has not renewed the Mandated Reporter Training, the licensee did not comply with the section cited above in that she has not renewed the Mandated Reporter Training course, which poses as a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with written instructions on how to locate the Mandated Reporter Training online. Licensee states she will complete this online course and provide LPA with a copy of her training completion certificate no later than 08/21/2025.
Based on interview and record review, the licensee did not comply with the section cited above in not having a current First Aid & CPR and expired on 1/2023 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will take the First Aid & CPR class on 2/18/23 and will submit a copy of a current certificate by 2/20/23 to the deparement by email.
