Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not have a documented fire drill which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will send the deparment of a completed fire drill conducted with the children by 3/22/22.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having an updated First Aid & CPR that expired on 3/18/22 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will complete the First Aid & CPR class on 3/30/22 and will send in to the department a copy of the completed certificate by 4/1/22.
Based on observation and interview, the licensee did not comply with the section cited above in that Licensee did not notify the department of proposed changed to convert the garage into daycare space which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee advised LPA that the construction began around March 1, 2022 and construction is set to end about April 31, 2022. Licensee will send in to the department an updated LIC279, facility sketch, and proposed changes in writing once construction is finished.
