CORTEZ, DELIA FAMILY CHILD CARE

Based on observation and interview, the licensee did not comply with the section cited above in not having 1 out of 3 adults residing in the home to obtain criminal record clearance prior to initial presence in the home which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee and Rosa Solis stated she has been residing at the home since July 2025. Solis stated she will obtain a criminal record clearance and submit TB test to the Department no later than 04/29/2025. Licensee stated she will write a statement on how she will ensure tenants residing in the home will obtain Criminal Record Clearance. Licensee stated she will also watch the Child Care Resource Video, "Background Check Requirements for Caregivers and write a summary.

Based on interview and record review, the licensee did not comply with the section cited above by not having documented or conducted fire drills and disaster drills at least once every six months which poses a potential health, safety or personal rights risk to persons in care.rsons in care.

Resolution: Licensee stated she will conduct and document a fire and disaster drills and submit a photo of the DIsaster Drill log to the SDCCRO 09/05/2025. Licensee states she will set reminders on her cell phone to get notifications to conduct Disaster drills every three months.

Based on observation and interview, the licensee did not comply with the section cited above in that Licensee did not ensure in reporting to the department using off limit room #2, converted garage to daycare room and bathroom #3 behind the new daycare room, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will send the LIC279, LIC999A, copy of converted garage permit, and proposed changes in writing to the department by 10/2122.
