Based on observation and interview, the licensee did not comply with the section cited above in that Licensee did not advise licensing when adding a daycare bathroom located in the daycare room, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee provided a letter of changes on this day 7/26/22, LPA will request a fire clearance for the bathroom addition. LPA observed bathroom to be safe for child care use pending fire clearance.
