Based on interview and record review, the licensee did not comply with the section cited above in that Licensee did not complete and provide a valid receipt of Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will complete the Mandated Reporter Training and submit a copy of the certificate to the SDCCRO by POC due date of 11/21/2025. Licensee states she will set reminders on her calendar and review her certificates to ensure she is in compliance with the Title 22 regulations.
Based on interview and record review, the licensee did not comply with the section cited above in that Licensee's Pediatric First Aid/ CPR expired October 2025 and did not provide receipt of a valid First Aid/ CPR training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will complete the Pediatric First AId/CPR training and will provide the date of her appointment and a copy of the certificate to the SDCCRO by POC due date of 11/26/2025. Licensee states she will set reminders on her calendar to ensure she is in compliance with the Title 22 regulations.
Based on observation, the licensee did not comply with the section cited above in Licensee did not ensure if having safe equipment for the daycare children. LPA observed a trampoline without the netting around it, which poses a potential safety risk to persons in care.
Resolution: Licensee states she will remove the trampoline by 11/30/22.
Based on record review, the licensee did not comply with the section cited above in Licensee did not ensure in having immunizations, (TDAP and Measels) for herself, poses a potential health and safety risk to persons in care.
Resolution: Licensee states she will have proof of TDAP and Measels by 11/30/22.
