Based on observation and interview, the Licensee failed to ensure that no children should be left without the supervision of a staff member at any time. Multiple children were observed going to the bathroom alone without a staff member escorting or supervising them. The helper affirmed that older children typically go to the bathroom by themselves, which poses a potential health and safety risk to children in care.

Resolution: The Licensee must ensure proper training on care and supervision is provided to all staff. The Licensee will submit a plan emphasizing the importance of children's safety through supervision and ensuring that no children should be left alone without active supervision. This plan will be sent to the Licensing office by November 11, 2022, to clear this deficiency.
