Upon arrival, LPA met with Center Director / Debbie A Leaf, Assistant Director / Christy Baumgartner and Infant/Toddler Instructional Coach / Shannon Burrington who guided LPA on a tour of the facility. There were 23 children with 10 staff observed present. LPA explained the purpose of today's visit is to follow-up on an incident that was reported to the Community Care Licensing (CCL) on 3/22/24. On 3/21/24, at approximately 3:42PM, during transition from the Toddler classroom 1 to the playground, Staff 1 did not conduct a proper name-to-face roll call, nor a visual sweep of the classroom, resulting in Child 1 being left inside Toddler classroom 1, unsupervised for approximately 3 minutes. After returning back to the classroom at 3:45PM, with a group of children, Staff 2 discovered Child 1 inside the classroom, sitting in the book area looking at a book. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted with Center Director / Debbie A Leaf and Appeals Rights provided.
