Based on record review, the licensee did not comply with the section cited above in LPAs did not observe the use of an infant sleep log that includes date, infants name and time of each 15-minute check which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall physically check on the infant every 15 minutes and document date, infants name and time of each 15-minute. Licensee will submit proof of documentation dated 08/09/2024 to 08/29/2024 via email to LPA Hernandez by POC due date.
Based on record review, the licensee did not comply with the section cited above in LPAs observed mandated reporter training for Licensee missing which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee shall complete the mandated reporter training and submit copy of current and valid certificate as proof of completion to LPA Hernandez via email by POC due date.
Based on record review, the licensee did not comply with the section cited above in Licensee did not have a copy of immunization for measles and pertussis. Both Licensee and adult daughter did not have TB clearance on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure copies of immunization's and proof of TB clearance will be in personnel file for the Department to review. Licensee will submit copies to LPA Hernandez via email by POC due date.
