Based on observation and record review, the licensee did not comply with the section cited above. The Mandated Reporter Training certificates for the Licensee and Staff 1 were expired and not current. Staff 2 was missing the Mandated Reporter Training certificate. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of current Mandated Reporter Training certificates for Licensee and Staff members 1 & 2 by the POC due date.

Based on observation and record review, the licensee did not comply with the section cited above. The Licensee does not have a current Pediatric First Aid and CPR certificate (expired: 7/7/21). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current copy of the Pediatric First Aid and CPR certificate to CCL by the POC due date.

Based on observation and record review, the licensee did not comply with the section cited above. Licensee was missing the LIC 627 / Consent for Emergency Medical Treatment and LIC 701 / Physician’s Report in the files of Children 1 - 3. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit completed copies of the LIC 627 / Consent for Emergency Medical Treatment and LIC 701 / Physician’s Report for Children 1 - 3 to CCL by the POC due date.

Based on record review, the licensee did not comply with the section cited above in 3 of 3 staff did not have a personnel file available for LPA to review which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, documents will be collected, scanned, emailed to LPA and submitted by POC due date.

Based on observation, the licensee did not comply with the section cited above. It was observed during inspection, a new house in construction located on the back of the house (off-limit), licensee did not report to RO, which poses a potential health, safety to persons in care.

Resolution: Licensee agreed to provide LIC624, with revised sketch floor plan and yard along with documents of the new construction such as permit and other documents..
