Upon arrival, LPA met with Director / Christina Lechner, who guided LPA on a tour of the facility. There were 17 children with 8 staff observed present. LPA explained the purpose of today's visit is to follow-up on an incident that was reported to the Community Care Licensing (CCL) on 1/24/24. On 1/23/24, at approximately 11:45AM, during transition from the Infant 1 playground to the classroom, Staff did not conduct a proper name-to-face roll call, nor a sweep of the playground, resulting in Child 1 being left on the playground unsupervised between 30 seconds to 4 minutes. After entering the classroom and settling the children, staff quickly realized that Child 1 was not in the room and Staff 1 went immediately to the yard to get Child 1. Child 1 was found alone playing in the yard. The following deficiency listed on the attached deficiency page is being cited in accordance with California Code of Regulations Title 22. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit. The Notice of Site Visit was provided and must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted with Christina Lechner and Appeals Rights provided.
