This requirement was not being met as evidenced by: The licensee did not comply with the section cited above in that Tiffany Chapon has been employed since 02/10/25 without a criminal record clearance or exemption which poses an immediate health, safety, or personal rights risk to persons in care. Which poses an immediate health, safety, and personal rights risk to children in care.
Based on interview, the licensee did not comply with the section cited above. Founder stated they completed the required lead testing but cannot find her documents. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation, interview, record review the licensee did not comply with the section cited above. LPA observed staff Taguhi Harutyunyan and Anastaiya Kimachenko who was fingerprint cleared but not associated with the facility. This poses/posed a potential health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above. Director was not able to provide a EMSA approved pediatric CPR/1st aid certificare for a staff member in the Preschool program, which poses/posed a potential health, safety or personal rights risk to persons in care.
(c) All outdoor and indoor passageways, stairways, inclines, ramps, open porches and other areas of potential hazard shall be kept free of obstruction.

(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
Buildings and Grounds. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement was not being met as evidenced by: During a walk through of the kitchen, LPA observed sharp knives left unlocked and accessible to children in the kitchen cabinet drawer, next to the sink. The kitchen is on limits to children in care as the children pass through the kitchen area in order to get to the bathroom nearest to the kitchen. During a walk through of the bathroom (near the kitchen area), LPA observed a Lysol toilet bowl cleaner left unlocked accessible to children underneath the bathroom sink. The bathroom (near the kitchen area) is on limits to children in care. This poses an immediate health, safety, and personal rights risk to children in care.
