Based on observation, interview, (record review. The licensee did not comply with the section cited above. At 9:00 am during the facility inspection, LPAs requested to see the fire and disaster drill log. The licensee stated they practice emergency drills with the children in care, but they have not documented the drill date/time at least once every 6 months, The last firedrill was conducted on 2/5/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will conduct a disaster drill as soon as possible and will submit documentation of the drill to LPA by the due date.
Based on observation, interview, and record review. The licensee did not comply with the section cited above. At 1:45 PM during document reviews, and interviewed, LPA observed the last firedrills was not document date and time, which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states, will conduct a disaster drill as soon as possible and will submit documentation of the drill to LPA by due date.

The licensee did not comply with the section cited above, At 1:45 pm, during staff file review and interview, staff #1 and staff #2 members did not have their mandated reporter training certificate expired 4/7/2024, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states, will submit the updated/current Mandated Reporter training certificate for Staff 1 and Staff 2 to LPA by the due date.
