Based on observation, interview, record review, At 10:00am during children’s file reviews, LPA observed Child #1 was missing copies of their immunization records in child’s file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will speak with child #1 parents to request a copy of the immunzation records. Licensee will submit a copy to licensing office by the due date.
Based on observation, interview, record review, Child #1 did not have LIC9227 Infant Sleeping Plan completed and filed in child’s file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated they will provide child#1 parent a copy of LIC9227 and have them complete the form. Licensee will submit a copy of the completed form to licensing office by the due date.
Based on observation, and record review, At 10:30am during children’s file reviews, LPA observed Child #4 and Child #5 were missing copies of their immunization records in child’s file. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated, they will ask parents again to submit copies of Child #4 and Child #5 immunization records and provide a copy to licensing department by the due date.
