Based on observation, interview, record review. The licensee did not comply with the section cited above, . At 1:00 p.m., during document reviews and interviews, both assistants stated that they didn't know where the fire drill papers were, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Assistant states she will conduct a disaster drill as soon as possible and will submit documentation of the drill to LPA by due date.

Based on observation, interview, and record review. The licensee did not comply with the section cited above. Around 1:53 p.m., another assistant (S2) arrived. Both Assistants stated the Licensee is currently out of the country and had left on 2/1/2026, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The assistant stated that they will contact the licensee and inform the Licensee about the Licensing visit today. The licensee will review the regulation about the Operation of Family Child Care, and write a statement acknowledging the regulation.

Based on observation, interview, and record review. The licensee did not comply with the section cited above. At 1:00 pm, during the records review and interviews, the assistant stated that she did not have LIC 9227 Individual Infant Sleeping Plan for Child #2 in the file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The Assistant stated, they will provide the LIC9227 form to Child #2 parents to complete and submit a copy to the licensing department of the forms completed by the due date

Based on, observation, interview, and record review. The licensee did not comply with the section cited above, During the records review, LPA did not see any record of licensee reporting to the department that they will be away from the facility for any period of time, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The assistant will contact the licensee and inform about the licensing visit today, 02/02/26. The licensee will contact the LPA via email within a week from now, by 02/09/2026, The licensee will review the regulation about the Operation of Family Child Care, and write a statement acknowledging the regulation.
