Based on observation interview, record review. The licensee did not comply with the section cited above. At 9:00am, LPA observation, and interview the licensees, they is allowing children to utilize one (1) bedroom located next to the kitchen and living room, not listed on the facility sketch LIC 999, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will email the new floor plan to LPA by the POC date. The licensee will notify the licensing office of any changes to the facility's physical plant.
Based on observation, iinterview, record review, the licensee did not comply with the section cited above, Staff #1 (S1) admitted that Staff #2 (S2) does not have Immunization, and provide evidence of a tuberculosis (T/B) result, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Staff #1 (S1) stated will send copy of Immunization, and TB test results from Staff #2 (S2) to LPA by due date.
